What federal law says
Section 1927(d)(2)(A) of the Social Security Act lets a state Medicaid program exclude agents when used for anorexia, weight loss, or weight gain from coverage. That is the legal basis for the split you see: Ozempic and Mounjaro prescribed for type 2 diabetes are covered like any other drug on the preferred list, while Wegovy and Zepbound prescribed for obesity are covered only in states that have opted in, usually with prior authorization tied to body mass index and a supervised weight-loss attempt.
In 2026 CMS launched the BALANCE model, a voluntary program in which CMS negotiates prices and coverage terms for GLP-1 drugs with the manufacturers on behalf of participating state Medicaid agencies. Medicaid coverage under the model could begin as early as May 2026, and states may join through January 1, 2027. The drugs listed include Mounjaro, Ozempic, Rybelsus, Wegovy, Zepbound and Foundayo. Which states have joined is not published in one place; your state agency can tell you.
Coverage can also turn on the indication on the prescription. Wegovy carries a cardiovascular indication, and some states cover it for patients with heart disease even where they exclude it for weight loss.
Children under 21
EPSDT does not override the drug-class exclusion, but a state that covers the drug for adolescents with obesity does so under EPSDT medical necessity.
What to ask your state plan
- Ask the prescriber to write the diagnosis the state covers; a diabetes or cardiovascular indication changes the answer.
- Check the state's preferred drug list, which is public, for the GLP-1 class and its prior authorization criteria.
- Find your state's agency, program name and phone on Medicaid by state.
