Almost never. There is no federal category for infertility treatment, fertility drugs are on the list states may exclude, and only a handful of states cover any of it. Federal rule: not required; rarely covered.
What federal law says
Federal Medicaid law does not list infertility treatment as a mandatory or optional benefit, and the mandatory family planning benefit is about preventing or spacing pregnancy, not achieving it. Section 1927(d)(2) specifically allows states to exclude fertility drugs from prescription coverage. The result is that IVF, intrauterine insemination and most fertility medication are outside Medicaid in nearly every state.
A few states have added limited fertility benefits, for example diagnostic testing or fertility preservation before cancer treatment, and state insurance mandates that require private plans to cover IVF do not reach Medicaid unless the state chooses to extend them.
Treatment of the underlying medical conditions that cause infertility, such as endometriosis, thyroid disease or polycystic ovary syndrome, is ordinary medical care and is covered.

- Ask whether the state covers diagnostic workup or fertility preservation before chemotherapy; those are the two places coverage sometimes exists.
- Clinic financial counselors know which state programs, if any, pay for part of a cycle.
