Three things to have ready before you call

Replacements ship to the address on file and don't forward.
Your benefit survives the phone
Losing the device never cancels your Lifeline enrollment. The benefit belongs to your household, not the handset. If a replacement quote doesn't make sense, two alternatives: put the free replacement SIM in any unlocked phone you can borrow or buy used (BYOD), or wait for your annual carrier-switch window and enroll with another provider that ships a new device. Keep using the number every 30 days in the meantime so the account stays active.

Only if you enroll as a new Tribal Lifeline or California LifeLine customer. Their own FAQ carries that limit in the asterisk. In Texas, non-Tribal Oklahoma and West Virginia their state pages say plainly that you receive a free SIM card and no phone. So for most applicants this is a bring-your-own-phone carrier, and you need an unlocked handset before you apply.

Yes. Their terms of service and privacy policy both name the entity as Boomerang Wireless, LLC doing business as enTouch Wireless. Federal Lifeline data carries listings under both names for the same company, which is why you may see the two used interchangeably.
