Three things to have ready before you call
Have your enrollment ID or the phone number on the account ready, know whether the device was lost/stolen versus defective (defective units are usually replaced cheaper or free under the initial warranty window), and confirm your mailing address in My Account first. Replacements ship to the address on file and don't forward.
Your benefit survives the phone
Losing the device never cancels your Lifeline enrollment. The benefit belongs to your household, not the handset. If a replacement quote doesn't make sense, two alternatives: put the free replacement SIM in any unlocked phone you can borrow or buy used (BYOD), or wait for your annual carrier-switch window and enroll with another provider that ships a new device. Keep using the number every 30 days in the meantime so the account stays active.

What states does Assist Wireless serve?
Four: Arkansas, Maryland, Missouri and Oklahoma. Their own state plan pages list exactly these, and USAC's federal carrier data agrees. Their About page also names Illinois, Louisiana, Mississippi, Pennsylvania and Texas as planned expansion, which is not the same as being live there. If you are outside those four, check your state guide for carriers that actually serve you.
Where are the Assist Wireless stores?
Almost entirely in Oklahoma, around 30 locations. Tulsa has seven, Oklahoma City four, Lawton two, and there are single stores across Altus, Anadarko, Bartlesville, Broken Arrow, Chickasha, Claremore, Duncan, El Reno, Enid, Midwest City, Muskogee, Norman, Okmulgee, Ponca City, Sapulpa, Shawnee and Tahlequah. If your documents are awkward, a scanned award letter, a name that does not quite match, this is worth the drive. A clerk looking at the paperwork beats three rounds of uploads.
What is the Assist Wireless customer service number?
1-855-392-7747 for existing customers, Monday to Friday 10am to 6pm Central, closed weekends. From an Assist Wireless handset you can dial 611. Enrollment questions go to a different line entirely, (888) 295-7937, which runs 9am to 7pm Central. Calling the wrong one is a common reason people give up.
What do you actually get on the Assist Wireless free plan?
Non-Tribal customers in all four states get 300 talk minutes, 300 texts and 4.5GB of data at $0. Oklahoma Tribal customers get unlimited talk, unlimited text and the same 4.5GB. If 300 minutes is tight, their paid tiers climb from $1 to $30 a month, and a $25 tier moves you to unlimited talk with 6GB.
Does Assist Wireless give a free phone?
Only on new Tribal enrollments, and only while supplies last. That is narrower than most carriers, and worth knowing before you apply. Non-Tribal customers at an Oklahoma store are quoted upgraded handsets for around $5 to $10. Everyone else can bring an unlocked phone, get a free SIM, and collect a switching credit for doing it.
Is Assist Wireless legit?
Yes, it is a genuine Lifeline carrier and appears in USAC's official list of Lifeline companies for its four states. One thing to read with open eyes: their own site publishes two different device return windows, 30 days in the terms and conditions and 7 days on the device help page. Get the window confirmed in writing before you accept a handset. And verify any carrier's status yourself on USAC's list rather than taking our word or theirs.
